HOW IT WORKS
CARE PLAN: The care plan includes the result of the assessment, recommendations and referrals for care options. We go into great depth to explain the details of the plan, what led to the recommendations, what the client can expect. Then we prioritize the needs list. COORDINATION OF SERVICES: Even when services are not available directly through us, we will arrange those services through other agencies. We find out what the client is able to do for him/herself, how other family members can be of assistance. Then we match this information to the priority list and to the client's economic abilities. After evaluation of all the data we help arrange for and monitor services. We will stay in contact with the caregiver and keep them informed on an as needed basis. All information is kept confidential in accordance with HIPPA. MONITOR CLIENT NEEDS/FOLLOW-UP VISITS: The first meeting establishes a baseline and the follow-up assessments are compared to the initial assessment to determine if any changes need to be made in the care-plan. FEES: Elder-Care Resources, Inc. provides a free phone consultation. Subsequent care management fees are billed on a fee for service basis. Fees are generally not covered by Medicare, Medicaid or other health insurance. and receives no fees from any facility for referral or placement. Top | ||||||
